None of our vans are allowed off-road and even rough roads can cause damage to our vintage Camper Vans. Don't get into situations that require fast acceleration. Visibility backing up is limited, use a spotter to guide you. Heavy vans can struggle to start on a steep uphill grade. "G. Polihale State Park is not allowed. Unimproved or dirt roads including Maha'ulepu beach are not allowed. GPS tracking will verify compliance. Driving in these areas or on similar unimproved roads will forfeit $500 from your deposit.

Heaven and Kama were super helpful and accommodating. The campervan needed some minor repairs (ie, fridge and stove not working) but they accommodated us by providing an ice chest and portable camping stove. The most important part - the engine - worked well in this campervan for its age.
Furthermore, their website does a really good job of explaining where you can camp on the island, and in person they are very helpful in explaining where you can and can't go, as well as the peculiarities of an older car.
This isn't specifically about the rental company; but overall Kauai is not the most campervan-friendly island so I recommend doing your research beforehand, especially about where non-4WD, low-clearance cars can go and actually stay overnight, as options are quite limited. Unfortunately I was not able to live my dream of waking up on the beach in a campervan. The campervan however does come with a tent which makes sleeping on the beach more possible. Its funny when you add up all the expenses, including campground fees, its actually the same price to stay in a cheap hotel and rent a normal car. If you rented a more expensive campervan you'd actually be paying much more to camp, so at least with this one you break even!
